COVID-19 SYMPTOM CHECKLIST

It is required that you check your child carefully for symptoms of COVID-19 each morning prior to sending them to school.  It is important that you notify the School Nurse of any illness during the school year, regardless of whether illness occurs at school or at home. Symptoms include:

  • Fever (100.0°F or higher), chills or shaking chills
  • Difficulty breathing or shortness of breath
  • New loss of taste or smell
  • Muscle aches or body aches
  • Cough (not due to other known cause, such as chronic cough)
  • Sore throat, when in combination with other symptoms
  • Nausea, vomiting, or diarrhea, when in combination with other symptoms
  • Headache, when in combination with other symptoms
  • Fatigue, when in combination with other symptoms
  • Nasal congestion or runny nose (not due to other known cuases, such as allergies), when in combination with other symptoms

If your child has any of the above symptoms, please keep them home and contact the school nurse for further guidance.

GRADE 7 HEARING, VISION AND POSTURAL SCREENINGS

As required by the Dept. of Public Health, all students in grade 7 are required to have vision, hearing and postural screenings along with height and weight. If a student does not pass a screening, a letter will be mailed home with details and a request for a private physician to follow up. If you would like your child to be exempt from a screening, please let me know and you will need to send in a current physical exam dated within the current school year.

GRADE 8 POSTURAL SCREENINGS

As required by the Dept of Public Health, all students in grade 8 will have postural screenings completed during the school year. If a student does not pass the screening, a letter will be mailed home with details and a request for a private physician to follow up. If you would like your child to be exempt from the screening, please let me know and you will need to send in a current physical exam dated within the current school year.

Grade 7 Requirements:  The MA Dept of Public Health requires that, BEFORE entering grade 7, all students must have:

- an updated physical exam dated within 1 year of entry into grade 7

- 1 dose of MenACWY (formerly MCV4)

- a Tdap booster

- 2 doses of Varicella

Grade 8 Requirement: The MA Dept of Public Health requires that, BEFORE entering grade 8, if they have not already gotten it for grade 7, students must have 1 dose of MenACWY (formerly MCV4)
